GP Training

The practice occasionally takes part in teaching students gaining experience in general practice. You may occasionally find a student sitting in during a consultation.  We hope you will understand and co-operate with this.  However if you would prefer to consult your doctor alone please let us know and we will be happy to respect your wishes.

Donation Account

The practice is hugely grateful for any donations recieved from patients, family and friends. Donations allow the surgery to purchase new equipment which can help towards more efficient diagnosis and treatment.

Should you wish to contribute towards such purchases, please feel free to contact the practice manager on your next visit. We thank you again for all donations we recieve.

Wheelchair Access

Access for most wheelchairs is possible at both surgeries and both have a WC which can be accessed by most wheelchair users

Patient Rights and Responsibilities

We aim to treat our patients courteously at all times and expect our patients to treat our staff in a similarly respectful way.  It is your responsibility to keep your appointments, inform us of your past illnesses, medication, hospital admissions and any other relevant details.

The practice operates a zero tolerance policy; the staff is here to help you. Our aim is to be friendly and as helpful as possible to all our patients.  If you consider that you have been treated unfairly and inappropriately, please ask the reception staff to contact the practice manager who will be happy to address your concerns.  Shouting, swearing and any form of verbal or physical threats will not be tolerated under any circumstances and patients who are abusive in any way will be removed from the patient list.  Please help us to help you, remember this is our place of work.

We respectfully request that if you are unable to keep an appointment you ring to cancel it as that slot can be offered to someone else.

Local Complaints Procedure

If you have any problem with any services you may comment in writing or in person to any of our professionals or members of staff. If the initial response is not satisfactory the practice manager is available and will try to resolve the problem.

Confidentiality

Your medical records are held in the strictest confidence.  Information is not passed on without your consent unless it is within the confines of the NHS, by legal framework, or is in the public interest.  Certain anonymous patient data may be shared for the purposes of public health and audit, research, teaching and training.   This practice is registered under the Data Protection Act.  It is a practice and legal requirement that all staff maintain confidentiality of patients? records.

All health professionals and staff of the practice have access to all clinical details on the practice system. All are fully aware of the need for confidentiality. We normally cannot give information to you about any other patient without their specific permission, no matter how close they are to you.

You have the right to see your own clinical record. If you require paper copies, there will be a charge to cover costs. Since medical records can easily be misinterpreted, the doctors would prefer you to make an appointment to discuss the records together.

Freedom of Information – Publication Scheme

The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme.  A Publication Scheme is a guide to the classes of information the practice intends to routinely make available.

Violence and Aggression

We are committed to taking all reasonable precautions to ensure the safety and well being of our employees and endeavor to protect them from physical and verbal abuse. Any incident will be reviewed by the partners and appropriate action taken, which may include removal from the practice list.

How Do I?

Register as a patient

To register as a patient you need to complete the registration form and a patient questionnaire these are both available from the surgery.   When these are completed hand them in to the Reception who will make an appointment with a GP for a New Patient Health Check.

Make an appointment

The Surgery phone line is open weekdays from 8.00am to 6.30pm. Please note that due to our practice half-day there is no doctor available on Wednesday afternoons. If you require a doctor urgently at this time, medical cover is provided by the Parc Glas Surgery from 12pm up to 3pm on 01407 840294 and the Llanfaelog surgery from 3pm up to 6.30pm on 01407 810313.

If you cannot keep a booked appointment, please let reception know as soon as possible so that the appointment can be used by someone else.

Repeat prescriptions

Please make requests in writing, and allow two working days for preparation and checking.  If you supply an SAE with your request, we can put your prescription in your envelope and send it through the post.

Obtain results

Please telephone the surgery to request your test results, if you require an appointment the receptionist will inform you of this and arrange an appointment for you.
