Register as a patient
To register as a patient you need to complete the registration form and a patient questionnaire these are both available from the surgery. When these are completed hand them in to the Reception who will make an appointment with a GP for a New Patient Health Check.
Make an appointment
The Surgery phone line is open weekdays from 8.00am to 6.30pm. Please note that due to our practice half-day there is no doctor available on Wednesday afternoons. If you require a doctor urgently at this time, medical cover is provided by the Parc Glas Surgery from 12pm up to 3pm on 01407 840294 and the Llanfaelog surgery from 3pm up to 6.30pm on 01407 810313.
If you cannot keep a booked appointment, please let reception know as soon as possible so that the appointment can be used by someone else.
Get medical help outside surgery hours
You can call 111 for free access to urgent out-of-hours care and round-the-clock medical support and guidance. This number is free to call and combines the services previously offered by NHS Direct Wales and the GP Out of Hours service.

Repeat prescriptions
Please make requests in writing, and allow two working days for preparation and checking. If you supply an SAE with your request, we can put your prescription in your envelope and send it through the post.
Obtain results
Please telephone the surgery to request your test results, if you require an appointment the receptionist will inform you of this and arrange an appointment for you.
